ARMS Medical Paperwork (Athletics) Please contact your team Athletic Trainer (AT) with any questions you may have regarding this paperwork or additional information as it relates to your physical or mental health. Keep in mind that your medical physical will take place and will be scheduled by your AT. More details to come!
